Preparing for Your First Visit

All new patient appointments must be scheduled by phone. When you call, our Scheduling Department will help you set up your child’s first visit.

To provide the best care, we ask that all new families submit a full copy of your child’s health records, including immunizations, before the appointment. Please bring a copy of your child’s immunization record to the visit.

We follow vaccine recommendations from the American Academy of Pediatrics (AAP). All patients are required to be up-to-date on immunizations by ages 2, 5, and 11. View our immunization policy.

New Patient Forms
We ask that all new patients complete the Patient Information Form and the Patient History Form found below and bring to your first appointment.

Completing these forms ahead of time ensures a smooth, efficient visit and allows our providers to focus on your child’s care.

Medical Records Transfer

If your child is transferring care to Advanced Pediatric Associates, we kindly ask that you request a copy of their medical records from their previous healthcare provider. Having these records allows us to ensure a smooth transition and continue your child’s care without interruption.

To request records, please download and complete our Authorization to Release Records.
 
Provide the completed form to your child’s previous provider and ask them to send the records directly to our central medical records department.

Please note: Most practices do not charge a fee when medical records are mailed or faxed directly to Advanced Pediatric Associates.
