Create a MyChart Account

Parents or legal guardians of children aged 17 or younger can set up a MyChart account through any of the following options:



• Sign up online now.

• Call 303-699-6200 during regular business hours to verify account information and receive an activation email, text, or letter.
• Ask a receptionist when you are in our office.

Important Notes:

• Teens aged 14 to 17 must complete a MyChart activation request card at any of our office locations to create their own account.

• Once a patient turns 18, they are considered an adult under healthcare privacy laws. At that time, parents or guardians will no longer have access—limited or otherwise—to the patient's MyChart account. The patient must activate and manage their own MyChart account moving forward.

🔐 Please note Parent/Legal Guardian Access to MyChart Account:

• Patients under 14: Parent/guardian has full access
• Patients 14–17: Parent/guardian has limited access
• Patients 18 and older: No access

What Services Does MyChart Offer?

MyChart is a convenient, secure online patient portal in partnership with Children’s Hospital Colorado. With MyChart, patients can:

• Schedule appointments for your child's well-care and sick visits, immunization appointments, or asthma follow-up visits
• Receive appointment reminders
• Submit school, sports, and other forms to our offices for completion
• View lab test results and some radiology reports
• View medications and request prescription refills
• Download immunization records for daycare, school, camp, or sports
• Communicate online with our offices
• View growth charts and health library
• Online bill payment
