Advanced Pediatrics offers many ways for patients to access information in an electronic format. Please see below for information regarding our online secure messaging, new patient forms, and downloadable diagnostic forms.
Online Secure Messaging
Advanced Pediatrics offers online secure messaging for updating patient information forms, and requesting prescription refills and copies of immunization records. All of the information you enter on these forms is securely encrypted to ensure your privacy.
Create a New Account
In order to take advantage of our online secure messaging you will need to do the following:
First: Create an Online Forms Account by providing your name and e-mail address, and creating a password to access your secure forms. If you have not yet created an online forms account, please click on the link below:

Second: Complete a separate Patient Information Form for each child that will be seen at our practice. The Patient Information Form collects demographic and insurance information necessary for communicating with you, completing online requests, and processing insurance claims. For your convenience, duplicate family information is automatically copied to each additional child that you add. If you have not yet created a patient information form for each child that will be seen at our practice, please click on the link below:

New Patients
We ask that all new patients "Create a New Account" by completing steps outlined in the yellow box above. By creating an online forms account (and completing a patient information form for each child that will be seen at our practice), you will be able to take advantage of our online secure messaging services (which includes convenient online requests for prescription refills and copies of your child's immunization records).
New Patients who choose not to create an online account will be asked to fill out paper registration forms at their first visit, and will not have access to online services such as prescription refills and immunization record requests.
In addition, we ask that you download, complete and bring the following Patient History Form to your first appointment.
Both the patient history form and the medical record transfer form require Adobe Reader to download. If you do not have Adobe Reader, you may download it by clicking on link below.
If you are transferring to Advanced Pediatrics from another health care provider, please download, complete and give the following form to your child's previous provider -- so that we may establish a complete medical record for your child.

Below you will find a number of diagnostic forms that you may be asked to complete prior to a visit with Advanced Pediatrics. These forms may be downloaded, printed, completed, and returned to our office.

Child Development
The "Ages & Stages" Developmental Screening Questionnaires will be provided at the preceding well care appointment for all well care visits between the ages of 2 months and 5 years. If you did not receive a questionnaire or have lost yours, we ask that you arrive 10 - 15 minutes early for your well care appointment so that you will have sufficient time to complete these important questionnaires. If you prefer, you may stop by our office in advance of your appointment and pick up a copy of the questionnaire to fill out at your convenience (prior to the date of your appointment).
